Clearer PHQ Guidance and Smarter Agency Checks

This release improves how Guardian guides applicants through Personal History Questionnaire questions and how investigators build Agency Checks lists. It also corrects several labels, validation indicators, and conditional questions so each workflow shows the right information at the right time.

PHQ Updates

This release includes several improvements to Personal History Questionnaire guidance, question visibility, and validation:

  • Configurable time frames: Supervisors can configure the required time period for select PHQ questions. For example, an agency can require a specific number of years of employment history. Guardian validates each response against the agency's setting and identifies missing information before submission.

    Handling Applicants' Previous Answers

Agency guide for supervisors and investigators: how applicants' previous PHQ answers carry over to your agency, why some answers don't arrive (time frame, contact period, quantity, and wording), and how to set time frames and minimum references in the Questionnaire Manager.

  • More relevant follow-up questions:
    • Guardian hides child-support and active-relationship questions when a related person is marked as deceased.
    • When an applicant has no supervisor to list, Guardian hides the supervisor-contact question. That question now reads, "Can we contact your supervisor(s)?" to account for jobs with more than one supervisor.
    • When a child is not a minor, Guardian hides the question asking who the child resides with.
    • When an applicant marks a child or sibling as a minor, Guardian no longer asks for that person's phone number or email address.
  • Legal-section validation: Required questions in the PHQ Legal section once again display red validation highlighting when information is missing. This includes validation after an applicant adds a repeatable entry, such as an additional incident.

More Focused Agency Checks Lists

Agency Checks lists now automatically include only local, municipal, and county agencies within the applicant's jurisdiction. State-level agencies are no longer added automatically, but investigators can still add a state agency when it is relevant to the case.

The first time an investigator opens an Agency Checks list after this update, Guardian displays a one-time notice explaining the change.

Clearer Removed-Entry Labels

The "View Removed Answers" button is now labeled "View Removed Entries." The related "Removed Answers" window is now labeled "Removed Entries." These labels appear consistently in the applicable Investigation sections.

Correct Question Numbers in Applicant Notifications

Important Notice messages regarding the applicant's PHQ now display the applicable question number instead of the "Qcurrent" placeholder. Only applicants see these notification messages; they don't appear in supervisor or investigator screens.

Thanks for reading, and thanks, as always, for partnering with us.
